Acute Respiratory Distress Syndrome

چکیده
Acute respiratory distress syndrome (ARDS) is a clinical syndrome of severe dyspnea of rapid onset, hypoxemia, and diffuse pulmonary infiltrates leading to respiratory failure. ARDS is caused by diffuse lung injury from many underlying medical and surgical disorders. The lung injury may be direct, as occurs in toxic inhalation, or indirect, as occurs in sepsis ( Table 268-1 ) . The clinical features of ARDS are listed in Table 268-2 . Acute lung injury (ALI) is a less severe disorder but has the potential to evolve into ARDS ( Table 268-2 ). The arterial (a) PO 2 (in mmHg)/FIO 2 (inspiratory O 2 fraction) <200 mmHg is characteristic of ARDS, while a PaO 2 /FIO 2 between 200 and 300 identifies patients with ALI who are likely to benefit from aggressive therapy. The annual incidences of ALI and ARDS are estimated to be up to 80/100,000 and 60/100,000, respectively. Approximately 10% of all intensive care unit (ICU) admissions suffer from acute respiratory failure, with ∼20% of these patients meeting criteria for ALI or ARDS.

متن کاملValidity of Spo2/Fio2 Ratio in Detection of Acute Lung Injury and Acute Respiratory Distress Syndrome
Introduction: One ofdiagnostic criteria for Acute Lung Injury and Acute Respiratory Distress Syndrome is pao2/fio2 (PF) ratio 300 for ALI or 200 for ARDS. This criteria requires invasive arterial sampling. Measurement of Spo2/Fio2 (SF) ratio by pulseoximetry may be a reliable non invasive alternative to the PF ratio. Methods and Materials: In a cross sectional study we enrolled 105 sample o...
متن کاملRhabdomyolysis and Acute Respiratory Distress Syndrome Secondary to Multiple Wasp Stings
Background: Wasp stings usually cause local reactions such as itching, urticaria, angioedema and anaphylaxis. Life-threatening complications following multiple wasp stings are relatively rare and unexpected. However, rhabdomyolysis and acute respiratory distress syndrome following wasp stings are possible. Case presentation: A middle-aged male worker was stung by a swarm of wasps all over his b...

Acute Respiratory Distress Syndrome (ARDS) is a clinical syndrome consisting of tachypnea, refractory hypoxemia, and diffuse opacities on chest radiographs after infection or trauma which ultimately leads to respiratory failure. The principles of treatment are based on patient care in ICU, mechanical ventilation and medical treatments. By using lower plateau, less tidal volume, higher positive ...
متن کاملHypersensitivity Reaction and Acute Respiratory Distress Syndrome in Pyrethroid Poisoning and Role of Steroid Therapy
Background: Pyrethroids are generally of low toxicity to humans, but in suicidal poisonings which are usually associated with ingestion of high doses, they lead to severe systemic effects. Case Report: A 30-year old woman presented to emergency department with a history of intentional ingestion of about 15 mL of prallethrin around 3 days earlier. She complained of shortness of breath along with...
متن کاملPredictive Factors of Acute Renal Failure in the Neonates with Respiratory Distress Syndrome
Background: Preterm birth occurs in a large number of pregnancies, and its incidence has been reported to be on the rise. Acute kidney injury (AKI) is a common complication in the premature infants with respiratory distress syndrome (RDS). The present study aimed to determine the predictive factors, clinical courses, and outcomes of AKI in the neonates with the clinical and radiological manifes...
